Here you will find all the forms that we have regarding telephones:

New Line: Use this form to order a new line

Move Line: Use this form when moving you extension from one room to another. Please remeber that when moving from one building to another a new extension number will be issued to you, as extensions cannot be transferred between buildings.

Take Over a Line: Use this form when taking over an extension that belonged to somebody else.

Service Level Restrictions: If you want to enhance the access levels on your extension (i.e. make national or international calls) you need to use this form.

Set Up Mailbox: If you want to activate the mailbox associated to your extension please use this form.

Reset Mailbox: Have you been locked out of your mailbox? Use this form to unlock it.

Fault Reporting: If you experience problems with you service use this form to communicate with us.
