These forms should only be used under the guidance of the Programme Office.

Please download, print, and follow the instructions. Please ensure that you return the signed form (signed by both relevant academic staff member and you) to a member of the Programme Office for processing.
Can't print? - Call into the office and pick up a paper copy from the front desk.

1. Repeat-Module-Time-Conflict-Registration-Form
If your repeat module clashes with something on your Semester 2 timetable please use this form. You will need to get the Module Coordinator's permission to over ride this clash, get the form signed and submitted to the Programme Office so that we can change your registration accordingly.

2. Pre-requisite-Over-Ride-Form
If you are repeating or resitting a module which is a pre-requisite to a next Stage module and have the Module Coordinator's permission to over ride this pre requisite, then you need to get this form signed and submitted to the Programme Office so that we can change your registration accordingly.

3. ME-Pre-requisite-Over-Ride-Form
If you are a new ME student and are unable to register due to pre-requisites, then you need to meet your Programme Coordinator and get permission to over ride the pre requisite. You then need to get this form signed and submitted to the Programme Office so that we can change your registration accordingly.

4. Module-Time-Conflict-Over-Ride-Form
This form is only to be used in limited circumstances and under the guidance of the Programme Office. Student are not permitted to over ride timetable conflicts with any modules which they are taking as a first attempt.

5. Summer School Pre-approval Form

If you have taken part in a UCD Summer School and wish to use credits that you have earned as recognition of prior learning against your degree programme, then you need to get this form signed and submitted to the Programme Office so that we can change your registration accordingly.
