Electronic Copies:

Data Dictionary    Description of Lifeways database. Listing of all tables, fields, constraints and connections.
2011 GP Form    A form sent to general practitioners asking for their willingness to participate in a data/sample collection exercise in 2012.
2011 Parents Questionnaire    Questionnaire sent to Lifeways mothers and fathers, November 2011. Questions concerning health, work, household, mental well-being, economic circumstances, and asthma in the Lifeways child.
2011 Contact Check    Form sent to Lifeways mothers to check contact details, dates of birth of all family members.
2007 Questionnaire    Questionnaire sent to Lifeways mothers. Questions covering a wide range of research areas: nutrition, socio-economic factors, mothers’ health, children’s health, development and behaviour.
Baseline Questionnaire   The baseline questionnaire. Questions concerned health, diet, exercise, consumption of alcohol and other substances, socio-economic factors. A modified version was sent to the Lifeways mothers with some extra questions covering consumption of vitamin, mineral or other food supplements during the course of their pregnancy.

Hard Copies:

Year 3 Grandparent Follow-Up    A form sent to grandparents' GPs asking for some details of their health.
Adult Health Record   A health "diary" kept by all Lifeways adults, recording contacts with health care providers, dental treatments, visits to complementary medicine practitioners.
Parent-Held Child Study Record    For a subset of Lifeways children the parents maintained a record of the child's contacts with health care professionals and of developmental milestones.
2002 Grandparent Exam   A clinical examination of Lifeways grandparents was undertaken in their homes. Anthropometric measurements were taken, some questions regarding health were asked and a blood sample was taken for analysis.
2002 Recruitment Form    A form collecting names and contact details of both Lifeways parents and all four grandparents.
Consent Form    The form used to gather consent for participation in the study.
