Elective Medicals

Elective Medical forms

Please call us on 01-716 3134 to schedule an elective triage appointment with the nurse. The nurse will then advise you how to proceed.

Bring your form and your vaccination records on the day of your appointment.  

The cost will vary depending on the form, but may cost €40 to €60.

Some elective applications will require extra vaccinations or blood tests. The nurse will arrange for you to have these. The additional costs are listed below

Remember to organise your application in good time, as there may be unavoidable delay getting records or awaiting blood test results before the form can be fully signed off
