This checklist is to help you gather everything you need but is not to be submitted without the required records of vaccines.
Please bring all available vaccine records of MMR and Hepatitis B vaccines and TB testing along with Photo I.D. (passport or driving licence only) to your Health Screening appointment.
Do I have records of the following vaccines?
MMR (Measles, Mumps and Rubella) vaccine X 2 doses:
If YES, please bring to your health screening appointment.
(Serology not accepted)
HEPATITIS B vaccines:
If you did not previously have this vaccine, the vaccine course (3 doses) will be provided by UCD Student Health.
If you previously had Hepatitis B vaccines, records of vaccination are required & you should bring them to your Health Screen appointment.
TB (Tuberculosis) blood test (IGRA/QuantiFERON/TSpot) within the last 2 years
OR
TB Skin test (Mantoux/Tuberculin skin test-TST) within the last 2 years
If YES, records are required & you should bring them to your Health Screen appointment
If NO, you may be offered to have it done in your Health screen appointment for an extra charge
COVID vaccine.
If you have had a Covid vaccine, please bring the record card with you to your Health Screen appointment.
