I would like to access my medical records
Access to your medical records:
You have the right of access to all the personal information held about you by UCD Student Health Service. You are entitled to access and obtain a complete copy of your Medical Records or a portion of the record/medical data on your Record. It is vital to protect the confidentiality of what is sensitive personal information to ensure that your medical records are requested only by you and/or your medical treating GP, Family Doctor or Medical Specialist.
Please download and fill out the Form to access your Medical Records as held by UCD Student Health Service 2023 and choose the following options as below. When complete and signed send to our email address (opens in a new window)student.health@ucd.ie with requested photo ID:
Option 1:
Hard copy of medical records to be collected by you (the student)
- Please note that you will need to provide an official photo-ID when calling to collect the hard copy of the records as well as your student ID (i.e. passport or driving licence).
- If you wish to nominate a third party to collect medical records on your behalf you will need to nominate them in writing at the time of application. (Your nominee will need to provide a Photo ID to confirm their identity on collection).
Option 2:
Hard copy of medical records to be sent to you (the student) or your GP by registered post
- Please ensure to attach a copy of official photo-ID i.e. driving licence/passport with your request.
- On the form include the postal address of your GP/your official postal address.
Option 3:
Transfer of medical records directly to your GP (via Healthmail)
- Please ensure to attach a copy of official photo-ID i.e. driving licence/passport with your request.
- On the form include the healthmail email address of your GP.
Option 4:
Transfer of medical records to you (the student) by secure email
- Please ensure to attach a copy of official photo-ID i.e. driving licence/passport with your request.
- Please note we will encrypt your medical records with a password which is your Student Number to ensure confidentiality and send to your ucdconnect email address.
Request your Immunisation records:
- Email (opens in a new window)student.health@ucd.ie stating that you are looking for a copy of your Immunisation Record. Please provide your student number & telephone number in the email.
- We will email your Immunisation record to you; this can take up to two weeks. This will be an encrypted email. You will need to use your student number to access the encrypted file.
Note on Encrypted documents from UCD SHS:
- All sensitive correspondence from UCD SHS is encrypted. This includes records; medical certificates; referrals, results and many other forms of medical correspondence. To open this encrypted email, you will need to use your student number as the password.
