Request to Create or Amend a DS/CS Job

The below form should be completed in full in order to submit a request to:

  • Setup a new DS or CS Job
  • Make a change to an existing DS/CS job, including:
    • Changing the name of the Job.
    • Change the Manager associated with the Job.
    • Change the Cost Centre associated with the Job. 
    • Extend the end date for the current Job. 

This form requires approval from the Head of School/Unit or College Finance Manager. If you are not an authorised personnel, the form will be routed for approval within the School/Unit.
