Before starting your audit, you must submit a Clinical Audit
Proforma for review and approval by your hospital’s Clinical
Audit Office.
This step ensures your project meets governance, data
protection, and quality improvement standards.
Prepare your application
Before submitting, make sure you have:
- Chosen a clear, relevant audit topic.
- Defined your standard and SMART objectives.
- Outlined your sampling plan and attached your audit tool/data collection form.
- Identified your sponsor (consultant, ADON, Registrar, or HoD) and stakeholders.
- Please ensure a Registrar is included in your sponsor list.
- Completed all sections of your proforma.
- Do not include any patient identifiers in either your proforma or data collection sheet.
- Reviewed the audit checklist & flowchart.

Await Approval
Each site has its own proforma - make sure to use the correct one. You must wait for formal approval before beginning any data collection. Once approved, you may proceed with your audit as outlined in your proforma.
If you are rotating through a site outside of MMUH & SVUH and would like to submit an application, please reach out to the network directly at intern.smms@ucd.ie for further direction.
